This notice describes the website consultation questionnaire, Facebook and Instagram contact forms, the separate email-draft form, and website operation. It does not replace any insurance privacy notice provided by Kaleb or an insurer when you apply for or purchase a product.

Your consultation questionnaire

The consultation questionnaire asks for your contact details, state, coverage interest, and general answers to help prepare a conversation. When direct submission is available and you choose to submit, these details are sent through this website to a private request database hosted by Supabase. Jones Health Coverage and authorized technical service providers use the information to handle your inquiry, maintain the service, and prevent abuse. Please do not enter medical records, diagnoses, Social Security numbers, payment details, or employee lists.

A saved-request confirmation means the request was stored. It does not confirm that a specialist has reviewed it or that an appointment has been booked. If submission cannot be confirmed, you can contact the business directly or use the separate Google booking link. Permission to respond to your inquiry is not consent to automated marketing calls or texts. No automatic transfer to Ringy is enabled.

Submitted requests include a request identifier and an abuse-prevention fingerprint derived from the network address, used to prevent duplicate requests and limit spam. When you use one of our tagged advertising links, we may save its campaign label with your request. We do not send your website questionnaire answers to Meta. We do not place questionnaire answers in advertising events or booking URLs. Supabase’s privacy policy describes its service practices. Contact Kaleb about retention, access, correction, or deletion of a submitted request; some business records may need to be retained where applicable requirements permit or require it.

Facebook and Instagram contact forms

If you submit a contact form on Facebook or Instagram, Meta processes the contact details and responses you choose to send and makes them available to Jones Health Coverage and authorized team members. We use this information to respond to your request and follow up using Meta’s tools or the contact details you provide.

Our new consultation contact forms request your name, phone number and email address. An older form asks about insurance status and timing, and historical responses may remain in Meta. Please do not submit medical records, diagnoses, policy numbers, financial details, Social Security numbers or employee lists.

Submissions through these new Meta contact forms are not automatically copied to the website’s private request database or website lead-email alerts. Submitting a form does not book an appointment, enroll you in coverage or provide consent to automated marketing calls or texts. Meta’s Privacy Policy describes its service practices. Use the contact details below for questions, corrections or deletion requests.

The separate email-draft quote form

The separate quote form prepares an email addressed to Kaleb Jones. That form does not send the email or submit its entries to the request database. Entries remain in the current page while you prepare your request and are cleared when the page is reloaded or closed.

When you choose “Open email draft,” your email app receives the draft. You review and send it there. Your email provider’s privacy practices then apply. You can also copy the request and send it yourself.

The fields are your name, email address, state, coverage interest, optional phone number, and optional message. They are used to compose the request you choose to send. Once sent, Kaleb receives those details in his email account to respond to your inquiry. Contact him about the retention, sharing, or correction of information in his business records.

Booking a consultation

Consultation links open Kaleb’s appointment booking page on Google. Google collects the details you enter there and shares appointment information with Kaleb to arrange your meeting. Review the booking page before submitting. Google’s privacy policy applies to that service. Booking is completed on Google, separately from the website questionnaire.

The private administrator calendar

If an authorized administrator connects Google Calendar, the private workspace uses read-only access to display upcoming events from the configured calendar. It retrieves event identifiers, titles, dates, times, and Google Calendar links. Private or confidential event titles are replaced with “Private event.” The connection does not create, change, or cancel events or alter the existing booking page. Event details are fetched for display rather than saved in the request database.

Google connection credentials and account identifiers are encrypted and stored privately in Supabase so the authorized server can maintain this connection. This website does not store the Google account password. The account owner can revoke permission in their Google Account’s third-party connection settings and contact Kaleb to request removal of the saved connection record.

What to share

Keep an initial request to your contact details, state, coverage interest, and general questions. Do not include medical records, Social Security numbers, payment details, or financial account information. Discuss any need for sensitive information directly with Kaleb through an appropriate channel.

Website operation

This website is hosted by Vercel, which may process your IP address, request information, and device or browser information to deliver and secure the site. See Vercel’s privacy policy. This website does not add advertising pixels or cross-site advertising trackers. A temporary preference in this browser tab remembers whether the logo introduction has played; it is not sent to us and does not store your quote details.

Private administrator sign-in uses a necessary, short-lived session cookie to protect the workspace. This cookie is unavailable to website scripts and expires within one hour. Signing out clears the browser’s session cookie. Connecting Google also uses a temporary, necessary cookie that expires after ten minutes to protect the authorization process. These cookies are not used for advertising tracking.

Basic action measurement

We record the type of action when a visitor opens booking, clicks a phone or email link, starts the quote steps, or opens or copies an email draft. For the consultation questionnaire, we also count page views, starts, progress to the needs and contact steps, submission attempts, confirmed saves, and submission errors. These are activity counts, not unique people, qualified leads, or completed bookings. The measurement request contains only an action label. It does not contain form entries, medical information, the current page URL, or a visitor identifier. These records are stored in hosting logs under our hosting provider’s retention settings; normal hosting request information may also be processed as described above. We use counts to understand interest, not to confirm bookings or sent emails. No tracking cookies are set.

Tracking preferences and outside services

Basic action measurement is skipped when your browser sends Do Not Track or Global Privacy Control. The site does not implement cross-site advertising tracking. Following an outside link or opening your email app brings you under that service’s privacy practices. The site currently has no embedded booking service or automatic lead transfer to Ringy.

Changes and questions about your information

Updates to this website notice will appear on this page with a revised date. To ask about information you have sent to Kaleb, request a correction, or raise a privacy concern, use the contact details below. Please do not send identity documents or sensitive health information in an initial email.

Coverage information

Content here provides an introduction to the services offered. It is not a policy, contract, personalized recommendation, or binding quote. Availability, eligibility, prices, benefits, exclusions, and guarantees depend on the insurer, product, location, and individual circumstances. Sending an inquiry or arranging a consultation does not put coverage in place.
